Amoré Orthodontic Aligners — Teledentistry Informed Consent
Effective: January 1, 2026 · This consent applies to at-home (teleorthodontic) care provided by Amoré Orthodontic Aligners, Farmington Hills, Michigan.
1. What teleorthodontics is
Teleorthodontics is orthodontic care delivered remotely using technology — including at-home impressions, photographs, dental records, and video or messaging with our team — instead of, or in addition to, in-person visits. Your treatment is designed and overseen by Dr. Jessica Delgado, a US board-certified orthodontist.
2. What to expect
You will complete a secure intake, provide dental records (including radiographs), and take impressions of your teeth with a kit we mail to you. Our orthodontist reviews your records and impressions to determine whether clear aligner treatment is appropriate and safe for you, and — if so — designs your plan. Free teleorthodontic check-ins continue throughout treatment.
3. Benefits
Convenience, access to a board-certified orthodontist regardless of your location (where we are available), and typically lower cost than traditional in-office treatment.
4. Limitations & risks
- Remote care has limits: some conditions cannot be safely treated with at-home clear aligners and require in-person evaluation or treatment.
- Treatment requires current dental records including radiographs; clear aligners are not appropriate for everyone.
- As with any orthodontic treatment, possible risks include discomfort, tooth or gum irritation, relapse if retainers are not worn as directed, and the need for additional or in-person care.
- Technology failures or incomplete information may delay care. Accurate, complete information from you is essential.
5. Not a substitute for in-person care when needed
If our orthodontist determines that in-person evaluation or treatment is necessary, we will tell you and help you find appropriate care. Teleorthodontic care does not replace regular visits to your general dentist.
6. Your records & privacy
Your protected health information is collected and stored on our HIPAA-covered practice-management platform and handled under our HIPAA Notice of Privacy Practices and Privacy Policy — not on this website.
7. State availability & licensure
At-home care is offered only in states where it is permitted and where we operate. We confirm availability by your state at intake.
8. Voluntary consent
Your participation is voluntary. You may withdraw from teleorthodontic care at any time (refunds are governed by our Refund & Candidacy Policy). You have the right to ask questions before agreeing.
9. Consent
By proceeding, you acknowledge that you have read and understood this consent, that your questions have been answered, and that you consent to receive orthodontic care via teledentistry as described. For patients under 18, a parent or legal guardian (18+) provides this consent.
